Registration Documents

Documents Required for Registration in Tift County Schools

  1. Eye, Ear & Dental and Nutritional Screening (EED Form 3300)

  2. Immunization Record (Form #3231)

  3. Certified Birth Certificate

  4. Student's Social Security Card

Address Verification (2 needed)

1)  A valid residential written lease or rental agreement, and a current residential utility bill (gas, electric, water, telephone, or cable). These documents must contain the name and address of the parent/guardian/legal custodian.

OR

2)   A current  residential property tax statement, mortgage statement or deed to the home, and a current residential utility bill (gas, electric, water, telephone, or cable). These documents must contain the name and address of the parent/guardian/legal custodian. 

THE FOLLOWING REQUIREMENTS FOR THESE LOCATIONS ARE IN ADDITION TO THE ITEMS LISTED ABOVE: 

Tift County Pre-K:

  • If you receive any of the following services we will need a copy of the card (Food Stamps, Medicaid, TANF, SNAP, PeachCare, SSI or CAPS)

  • To be eligible for Pre-K for the 2023-2024 school year, your child must be 4 years old on or before September 1, 2023. (Birthdate must fall between September 2, 2018 - September 1, 2019)

Tift County High School: 

  1. Previous school’s attendance and discipline report

  2. Previous school’s transcript of credit earned and grades
