Kindergarten students will need to come with parent at the time of registration for a short kindergarten assessment.

Tift County Health Department Certificate of Eye, Ear and Dental Form (Form #3300)
Students New to Tift Schools: Tift County Health Department Certificate of Immunization (Form #3231)
