Use this checklist to guide you through your first few days of employment. You will need to have your picture ID (e.g., driver's license or passport) and social security card on your first day of employment. 
Please print this page for your records:

• Payroll Information Sheet – Yours to Keep. For Informational Purposes Only.
• I-9 Form – Complete and return
• W-4 Form- Complete and return.
• G-4 Form – Complete and return.
• Worker’s Comp Acknowledgement Form – Complete and return.  Please report all job related injuries to your immediate supervisor.
• State Security Questionnaire- Complete and return.
• Drug Free Policy Form- Complete and return.
• Drug Free Workplace Policy – Yours to Keep.
• Verification of Professional Employment Form – For certified staff only.  Complete and mail to all previous systems you worked in for verification.  Please make copies if you have worked in more than one system. 
• Direct Deposit Enrollment Form- Please complete and attach a voided check if you would like to participate.
• Paycheck form:  Complete and return. (13 Paychecks -For teachers who will not receive a check from a previous school system in August)
• SHBP Enrollment/Transfer OR Declination Form – Please complete only one, but return both. 
• Health Insurance Rates
• Attached is a New Employee Health Plan Decision Guide to help you with your decision.

You will meet with an insurance broker to discuss other voluntary benefits. You may view these benefits at www.mytiftbenefits.com.
